Welcome! Are you in need of a new PCP? Great! We're here to help. We are affiliated with MDVIP and work with most insurance companies, offering services that surpass the norm. Please note that this program requires an annual membership fee, but you'll soon see how we stand out from traditional primary care offices.
MDVIP was originally established in 2000, the network now spans 44 states, comprising 1,100 primary care physicians dedicated to making a difference one patient at a time. Corrales Family Practice partnered with MDVIP in August 2012, a decision for which we are forever grateful. MDVIP enables Dr. Thal to practice in a way that fosters longevity, promotes a healthy patient-physician relationship, and facilitates earlier disease identification for life-saving diagnoses.

Two visits are included. One is Prewellness and the second is Executive Wellness.

FAQSWhat services are included: There are two visits included with the MDVIP Program. The first is the prewellness and the second is the executive wellness visit. See above for what each visit entails. What services are billed to insurance? Any visits outside of the two listed above. Do I start with the Prewellness visit? No. All new patients start with a “New patient Consult” it is a 1-hour establishment visit. We will request records in preparation for the visit. We will review records and baseline medical history. Patients must be an active MDVIP member and this visit is billed to insurance. Does Corrales Family Practice do “meet and greets”? No. We no longer offer complimentary meet and greets. At Corrales Family Practice, we are confident that we will exceed your expectation. In order for us to be able to provide the type of care we believe you deserve; we ask for you to enroll and allow us to do what we do best. Will I receive a refund if I decide it is not for me? If after the first appointment we do not meet your expectations, please contact the Manager. All patients are entitled to a pro-rated refund if cancellation occurs before the prewellness appointment. Once prewellness and wellness are performed, the patient is obligated for the entire contract which is 1 year and $2100. Is Corrales Family Practice owned by MDVIP? No. We are a privately owned practice. We partner with MDVIP to provide a personalized wellness program. Are same day/next day appointments for all medical concerns? No. Same day/Next day appointments are for urgent matters. The staff at Corrales Family Practice will ask a series of questions to be able to prioritize the urgency of your medical concerns.

If you're ready to begin, please fill out the medical record request form below. We would like to request records from any physicians you've seen in the last 2 to 3 years, including specialists, urgent care/emergency room, and primary care. The form is located below. Please note that each facility does require it's own form.
Once the form is filled out, please send it to kgonzales@cfpnm.com. We are eager to schedule your first appointment. Please provide your availability for the initial 1-hour establishment appointment. Once the appointment is scheduled, please enroll with MDVIP by clicking “get started today”. We are looking forward to meeting you!
